- Copy of driver license or other picture I.D. with social a security number.
- Copy of last 2 years full income tax returns with W-2 statements and attachments.
- Copy of 30 days of recent paycheck stubs.
- Copy of 3 months bank statements.
- Name, address, account numbers, balances and monthly payment.
- Voter Registration
- Social Security award letters, evidence of Disability income or letters of entitlement on other non-employment income.
- Names and addresses of each employer for the past 2 years.
- Name and address of present and all previous landlords for the last 24 months.
- Divorce decrees, community property settlement and evidence of child support payments and/or alimony payments, if applicable.
